Consent Forms

I'm a GP and was always taught to use consent forms when I'm doing oral surgery. I'm hearing more and more though that they should be used more often. What is the rest of the community doing out there?

I've attended several risk management seminars and my impression was that you're not overdoing it if you have consent forms for endo, surgery, perio, even a signoff on dentures and veneers that the patient agrees to the shade and arrangement, etc. before going to final.

This more a comment on document what you say rather than consent forms. I had a case where I explained to the patient that a Valplast partial would cost more than the insurance company would pay for the cast partial; you're all familiar with the alternate benefit... Everything was fine until I billed the patient for the difference. After a lot of hassle, having the patient sign their chart making them responsible for the difference kept me out of court.

Some malpractice insurance companies have a risk management kit that includes boilerplate consent forms for all the dental disiplines
